Case Management is a collaborative process of assessment, planning, facilitation and advocacy for options and services to meet an individual's health needs through communication and available resources to promote quality, cost-effective outcomes.
Licking Memorial Inpatient Case Management
We are a multi-disciplinary team whose goal is to facilitate a patient's treatment plan, develop and support care transitions, and work towards reducing the financial burden for our patients and families.
Our RN Case Managers will certify our patients are in the correct status when hospitalized, ensure the plan of care is executed efficiently, provide resources, and safeguard payment for their services.
The Social Workers assist patients to transition to the next level of care successfully by securing resources such as home health, transportation or equipment, while other patients may need help with placement in skilled nursing facilities, acute rehabilitation, mental health facilities or other specialty care areas.
Our dietitians evaluate and treat patients' nutritional needs and provide education.
Community Case Management
Community Case Management monitors and plans services which may be necessary for our patients healthcare needs. Our staff helps patients coordinate care and services, serves as information specialists, assists in independent healthy living and supports patients to make responsible health care decisions.
The Community Case Management team includes:
