Sports Screening Program
Licking County students entering grades 7 through 12 who participate in sports, marching band, and C-TEC’s Criminal Justice, Fire, and Physical Therapy programs are invited to attend the LMHS Sports Screening Program at no cost.

The 2022 LMHS Sports Screening Program is complete. If your child needs a sports physical for the 2022-2023 school year, contact your family physician or pediatrician to schedule a health screening. Please check back next year for the 2023 Sports Screening Program.

Additionally, any student who requires a sports physical must provide a completed Ohio High School Athletic Association (OHSAA) pre-participation physical evaluation (PPE) form, a PrivIT profile, or your school's equivalent - signed by a parent or guardian - at the time of check-in.
