Sports Screening Program
Middle and high school students in Licking County entering grades 7 through 12 are invited to attend LMHS' Sports Screening Program at no cost. Dates for the screenings are available in May.

Registration is required in advance and limited to the first 80 participants each night of the program. To register, click the link below. After registering, you will be redirected to a packet of forms that must be printed out and completed prior to your arrival at the Sports Screening Program.

The 2018 LMHS Sports Screening Program is complete. The registration link for the 2019 program will be available in May. If your child needs a sports physical for the 2018-2019 school year, contact your family physician or pediatrician to schedule a health screening.

Additionally, any student who requires a sports physical must provide a completed Ohio High School Athletic Association (OHSAA) pre-participation physical evaluation (PPE) form, a PrivIT profile, or your schools equivalent - signed by a parent or guardian - at the time of check-in.
