Community Case Management monitors and plans services which may be necessary for our patients' healthcare needs. Our staff helps patients coordinate care and services, serves as information specialists, assists in independent healthy living, and supports patients to make responsible healthcare decisions. Community Case Management provides the following services:
Diabetes Self-management Education Support Classes
Our classes, which are recognized by the American Diabetes Association, are designed by our registered nurses, dietitians and pharmacists, and teach patients how to care for themselves on a daily basis. Registration and physician referral are required.
A series of two, 3-hour classes are required to complete the program. Classes are offered twice a month.

Nutrition Counseling
One-on-one nutrition counseling is provided by a dietitian to help with meal planning for disease management.
Glucometer and Insulin Counseling
One-on-one, hands-on counseling is provided by a nurse. Education focuses on using a glucometer and how to properly inject insulin.
Healthcare Coordination
Our highly skilled nursing staff provides home visits to offer disease process education and monitoring as well as medication education.
Community Screenings (includes blood pressure clinic)
Offered the second and fourth Thursdays of each month at C-TEC
