If you suspect that someone has used your information to get medical services, you should follow the instructions on the Federal Trade Commission’s (FTC) website located at https://www.identitytheft.gov/, to report ID theft and to get a recovery plan. Follow all directions provided by the FTC, including whom to contact if you suspect identity theft. Further, you should complete the following steps with Licking Memorial Health Systems (LMHS):

Request copies of your LMHS medical records by completing the Release of Medical Information Form
Review your medical records, and report any errors or mistakes in your medical records by contacting the following:
