Licking Memorial Health Systems - Measurably different...for your health
Billing Information
Licking Memorial Health Systems (LMHS) does not deny care based on the inability to pay, and discounted programs are available.

Licking Memorial Patient Financial Services manages the billing for Licking Memorial Health Systems (LMHS).  Effective February 27, 2021, patients receiving care from providers employed by LMHS no longer will receive two separate billing statements.  The new consolidated billing statement will provide patients with concise information regarding medical charges for both Licking Memorial Hospital (LMH) and Licking Memorial Health Professionals (LMHP).

Statements will list charges for services from LMH inpatient and outpatient facilities, as well as LMHP physicians, with the exception of the LMHP pathologists. You may receive statements from other providers in our facilities who are not employed by LMHS, such as consulting physicians or ambulance transportation providers.

For questions regarding your bill, please contact us at (220) 564-1500.  The Patient Financial Services Department is open Monday through Friday, 7:30 a.m. to 5:00 p.m.  Payments and correspondence also may be sent to this address:

Billing FAQs
How do I get a copy of an itemized bill?
You may call Patient Financial Services at (220) 564-1500 to request an itemized bill.

Will LMHS accept my insurance?
LMHS accepts Medicare, Medicaid, and other major insurance carriers. Please contact your insurance company or employer for the specific requirements of your plan.

Will LMHS bill my insurance for services rendered?
Yes.  As a courtesy to you, LMHS will send a claim to your insurance company for consideration of coverage. If we do not receive payment from your insurance company within 60 days, we must look to you for payment of the bill. Your insurance policy is a contract between you and your insurance company.  Communication with your insurance company is highly recommended.

Will LMHS bill my healthcare sharing ministry?
Maybe.  If you are a member of a healthcare sharing ministry and do not have health insurance you should tell registration that you are self-pay.  We will send your bill directly to you so you can provide your bill to your healthcare sharing plan for reimbursement.  However, in the event that we directly bill your healthcare sharing ministry, you are responsible for any unpaid portion of your bill.

Does LMHS accept credit cards?
Yes.  Visa, MasterCard, American Express and Discover are accepted forms of payment.  You also may pay with cash, check, or money order.

How can I check on the status of my account?
If it has been at least 30 days since your date of service, contact your insurance carrier for claim status. If questions remain after speaking with your insurance company, contact Patient Financial Services at (220) 564-1500.

What should I do if my insurance company denies a claim?
If you feel the claim was denied in error, contact your insurance carrier for instructions on how to appeal the decision.  Patient Financial Services also should receive an explanation of benefits (EOB) from your insurance company stating the claim was denied.  This information will be reflected on your account, and you will receive a statement requesting payment in full.

How will I know if my LMHS bill has been paid?
Your insurance company should send you an explanation of benefits (EOB) indicating they have processed the claim.  The EOB will show any payments made to LMHS and the amount that is your responsibility.  You will receive a statement from LMHS once all insurance claims have been processed. This statement will reflect all insurance payments, adjustments, and the balance remaining. Any remaining balance is your responsibility.

What if I cannot pay my bill?
LMHS offers monthly payment plans to patients who are unable to pay the amount due in full.  Payment arrangements may be established in MyChart or by calling Patient Financial Services at (220) 564-1500.

Patients who earn up to 250 percent of the federal poverty guidelines may be eligible for financial assistance.  Applications are available here or through an individual’s MyChart account.  An application also may be obtained by contacting Patient Financial Services.
