Elizabeth W. McIntosh, M.D., received her medical degree from State University of New York Upstate Medical University in Syracuse, New York.  She completed a family medicine residency at Grant Medical Center in Columbus.  Dr. McIntosh is board certified in family medicine.

Darci J. Hartshorn, APRN-CNP, received her Bachelor of Science in Nursing degree from Capella University in Minneapolis, Minnesota, and her Master of Science in Nursing degree from Northern Kentucky University in Highland Heights, Kentucky.  She is certified by the American Academy of Nurse Practitioners.
