In an effort to increase your CME value and in appreciation of your valuable time, we are designing CME to qualify for both
AMA PRA Category 1 Credits™
and Maintenance of Certification (MOC) credits. Currently we offer MOC credits for each Multidisciplinary Cancer Care Conference for the following boards.
ABIM MOC Recognition Statement – Internal Medicine
Medical Knowledge
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn up to 1 Medical Knowledge MOC points in the American Board of Internal Medicine's (ABIM) Maintenance of Certification (MOC) program. Participants will earn MOC point’s equivalent to the amount of CME credits claimed for the activity. It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of granting ABIM.
ABOHNS Continuing Certification Recognition Statement – Otolaryngology – Head and Neck Surgery
Successful completion of this CME activity, which includes participation in the evaluation component, enables the participant to earn their required annual part II self-assessment credit in the American Board of Otolaryngology – Head and Neck Surgery’s Continuing Certification program (formerly known as MOC). It is the CME activity provider's responsibility to submit participant completion information to ACCME for the purpose of recognizing participation.
ABPath Recognition Statement – Pathology
Lifelong Learning Part II
Successful completion of this CME activity, which
includes participation in the evaluation component, enables the participant to earn Lifelong Learning Part II in the American Board of Pathology’s (ABPath) Maintenance of Certification program. Participants can may earn up to 1 MOC points.

Participants must complete the following to be awarded MOC credits.
Attend session(s).
Participate actively in case discussions.
Complete the evaluation with MOC attestation.
Complete annual questionnaire 1910-DOC83 and return to Jessica Adamson, CME Coordinator at
jadamson@lmhealth.org
or fax to (220) 564-4012 on or by December 24, 2021.
