Community Case Management

Community Case Management is a collaborative process that assesses, plans, implements, coordinates, monitors and evaluates the needs and services that may be necessary to meet your needs after hospital discharge. Licking Memorial Hospital provides this service without cost to the patient and family.

  • Community case managers include registered nurses who work with your physician to assist you in meeting your health care goals by:
  • Coordinating needed care and services provided by health care professionals and the many agencies that provide community services
  • Communicating with others in the health care system, serving as an information channel to you and on your behalf
  • Educating you and your family about how to prevent complications and how to promote healthy living
  • Supporting and empowering you to make appropriate and responsible health care decisions, to gain increased competence and independence when it comes to managing your own health
  • In addition to the registered nurses, the community case management team includes:
  • Social worker
  • Dietitians
  • Fitness coordinator
  • Pharmacist
  • Community case managers have multiple roles, which include being a consultant, advocate and motivator.
  • You may contact Community Case Management with questions by calling (740) 348-4915. Monday through Friday from 7 a.m. to 4:30 p.m. You may leave a confidential voice mail message.
  • Community Case Management referrals can be made by:
  • Physicians
  • Registered nurses
  • Other health care staff
  • Community agencies
  • Family members
  • Also, please note:
  • Your primary physician must agree and order Community Case Management services.

    • A Case Manager can assist you in obtaining this order.
  • You must sign a consent to receive Community Case Management services.
  • You must participate in the development of a plan of care with specific goals and you must work toward achieving those goals.
    • Your physician will be advised of your progress toward these goals.
  • Community Case Managers supervise and direct a number of community support groups and programs, including:
  • Diabetic Class Series (a four-week series)

    • Four Tuesdays per month from 1 to 3 p.m.
    • Four Wednesdays per month from 1 to 3 p.m. or 7 to 9 p.m.

  • Healthy Lifestyles Road Show (includes blood pressure clinic)
